Transition from one state to another produces two identical tasks for the same process

Description

There is a product introduction workflow that is at first linear, i.e. there are no forks and joins in the first part of the process definition. (See attached process definition.)

Sometimes, not always, the transition from state S_2_1_PflegeBasisdatenAnlage to state "S_2_1_PflegeBasisdatenWait" creates task "S_2_1_PflegeBasisdatenWait" twice. This means, you have two identical tasks in the worklist for the same process where you should only have one!

Attached process definition is a modified version. I deleted the company's name and some personal information like email addresses.

NOTE that since GFW-88 Grailsflow 1.0 uses first node as start node so process definition should be adjusted.

Problem occured on productive system of our customer. I have a DB dump made of the current state that can be seen on our demo system.

productId of the corresponding process is "4F0054630A", you can filter this productId in the worklist.
The state is not the same as in the "old" error description, this time the state "S_1_2_TeilenummerAendernBestaetigen" exists twice.
